Complete the top three lines, then the bottom two lines. Leave the rest of it blank. After printing it out, sign and date the form, and fax it to Esquire Titling immediately so that it won’t delay us processing your CVR request.

However, please mail us the ORIGINAL form so we can mail it to the DMV to keep on record.
